An active choice form is a form that employers provide to new employees so they can indicate if they intend to join a union.
In 2019, the Ministry of Business Innovation and Employment (MBIE) mandated active choice forms through S62A of the Employment Relations Act 2000.
If you have started a role that is covered by a CEA (collective employment agreement) between your employer and a union, you can complete this digital form. You will be given the choice to join or decline E tū Union membership. Should you decline, you can let your employer know and you will be covered by an IEA (Individual employment agreement). If you do not wish to provide your details to E tū, do not complete this form.
Why join a union?
Unions support employees in the workplace by acting as an advocate for them collectively (and with the consent of the employee, individually). Unions work with employers to make collective employment agreements – these are agreements between employers and registered unions that cover employees in the employer’s workplace. They also help employees with information and advice about work-related issues.

If you choose to join E tū, you will be prompted to complete a Union membership application form. Once your application has been completed, will will confirm your membership with your new employer, and you will have access to the collective employment agreement at your workplace.
Completion of this form authorises your new Employer to deduct and pay Union membership fees on your behalf.
